Implementation, adaptability, and sustainability are essential to creating meaningful change in clinical outcomes. This is often the most difficult part of any translational or evidence-based project. The Iowa Implementation and Sustainability Framework offers a method to achieve your goals and 81 different implementation strategies within the four stages of implementation based on empirical evidence.

Use the Iowa Models evidence-based recommendations to create an implementation and sustainability plan for your proposed scholarly project. Look at Figure 2 (page 4) and Figure 6 (page 16) in Cullen, et al. (2022) (in ) for what to consider. Your plan should include well-developed paragraphs for each of the following:

1. Your evidence-based intervention and evaluation of how you will create awareness and interest in your project in your unit/organization (Phase 1 of the model).
2. Steps you will take in your organization to build knowledge and commitment to your intervention (Phase 2).
3. Plan to promote action and adoption of your intervention (Phase 3).
4. Ways in which you will make your intervention/practice change sustainable after your project has completed (Phase 4).
5. An appendix with your plan in table format with short statements based on Figure 6 (Cullen et al. p. 16). Use the

You must cite any authors from evidence you used to support your plan. However, you do not need other references unless you use literature in addition to Cullen et al. (2022). Rather than seeking out literature, thoughtfully approach your strategy and plan, focusing on possible barriers during the implementation phase and how those will be addressed.
