Instructions

1. Identify and analyze an opportunity for improvement for any processes in your healthcare organization (i.e. falls, pain, patient satisfaction, education, etc.). Introduce the issue in the introductory paragraph of your paper. Include how addressing this issue fits the mission and vision of the organization.
2. Analyze internal and external data to support the opportunities for change- what data is driving your decision? What is your goal? Use scholarly articles to support the scope and importance of the issue.
3. Select and discuss a quality improvement model of your choice, and describe how you would use this framework to implement an evidence-based solution to the issue.

6. EB solution- what evidence exists to support this intervention?
7. Key stakeholders (in both the microsystem and macrosystem)
8. Method of evaluation (short and long term)
9. Plan for sustainability
10. Select and discuss a change theory that you would utilize to help facilitate the quality improvement initiative and to sustain the change. Does the organizational structure within the organization foster or hinder this change?

Paper Requirements:

• The paper should be 6-8 pages in length (excluding title and reference page/s)
• A minimum of 5 scholarly resources utilized to support discussion within body of paper.
• APA formatting required including the use of level headings to organize body of paper (suggestion organize the paper based on requirements outlined above).
