Unit 4 Case Study
You are a newly licensed registered nurse (RN) working on a busy medical-surgical unit. During a busy shift, a colleague administers the wrong dosage of a high-risk medication to a patient with multiple chronic conditions. The error was discovered shortly afterward and reported promptly, and the patient fortunately did not experience long-term harm. The patient and family were notified of the error by the physician.
Upon investigation, it was revealed that the barcode medication administration (BCMA) system had been temporarily disabled because of a software update. Additionally, the unit was understaffed, and recent policy changes at the state level have limited overtime, making shift coverage difficult. The nurse involved followed hospital policy in documenting and reporting the error but was later reprimanded by the nurse manager, leading to concern from other staff who might otherwise report near-miss incidents. As a new nurse, you are aware of National Patient Safety Goals (NPSGs) (e.g., medication safety and communication during transitions) and the Agency for Healthcare Research and Qualitys (AHRQs) Culture of Safety. You are also aware that there has been prosecution of a nurse recently who made a medication error, despite the nursing profession supporting a just culture and shared accountability of system errors. You are now wondering whether nursing is the right profession for you.

- Begin by restating the case study in full, using your own words to demonstrate comprehension and contextual understanding (as if you personally experienced the situation).
- After completing Phase 1 (Description), continue with Phases 26 of the Gibbs Reflective Cycle. In addition to identifying the domain, competency, and concept in Question 1, you are also required to include these elements in Phase 6 (Action Plan), where applicable.
It is understood that not every competency may apply to Phases 1 or 6; however, by the end of the full analysis, all required domains and competencies from the rubric must be addressed.
