Scenario
The administrator of the hospital where you work voices concerns about the increased incidence of CAUTIs (Catheter-Associated Urinary Tract Infections) over the past quarter. The director of nursing (DON) states that the hospital has a policy that was developed to reduce the incidence of CAUTIs and asks you to provide suggestions for a quality improvement initiative that will enforce compliance with the current policy. As the Director of Education, you are aware that UTIs are the most common type of healthcare-associated infection, according to the National Healthcare Safety Network (NHSN). You advise the committee that national statistics report approximately 75% are associated with a urinary catheter and that these infections can be reduced with education (CDC, 2020).
You have been asked by the committee to create Quality Improvement guidelines for staff education that focus on the prevention of Catheter-associated UTIs (CAUTIs) and other types of healthcare-associated infections. During the next quarterly meeting, you will present an education plan for an in-service to all healthcare personnel who care for patients with catheters and an in-service for hospital leadership. Utilizing the evidence-based guidelines, develop an in-service that presents procedures for urinary catheter insertion, maintenance, and removal.
In addition to an education plan and agenda for the in-service, create an evaluation tool or use an existing evaluation tool that will measure plan effectiveness, and recommend a current or emerging technology that will support safe and effective implementation of the changes.
Instructions
Part I: Staff In-Service
Using the , create an education plan for an in-service for facility nursing staff that includes procedures for insertions, maintenance, and removal of urinary catheters to avoid CAUTIs.
Create an agenda for staff that includes quality improvement guidelines for staff education that focus on the prevention of CAUTIs. Include:
- Time for an introduction
- Time for review of the new policies related to prevention of CAUTIs
- Hands-on time to learn and demonstrate the procedures for insertions, maintenance, and removal of urinary catheters to avoid CAUTIs
- Create an introduction including the following:
- Describe why a policy change is necessary based on current evidence to improve outcomes, using the QI data above to support the change.
- Describe why epidemiology is so essential (Refer to the comparative data from the Center for Disease Control on CAUTI infections. Use this data as supportive data to provide evidence of the epidemiologic and global need to implement the initiative in your institution. Describe how these statistics are collected and how you will use your electronic health record to report and analyze data for your institution to meet national epidemiological goals of decreasing CAUTI infections.)
- Describe how this intervention will impact global initiatives for improvement of CAUTI infections
- Describe how health disparities influence this as a local issue in your facility.
- Create an evaluation tool or identify an existing evaluation tool that will measure the effectiveness of the plan.
Part II: Leadership In-Service
Create an in-service for leadership leading up to the staff in-service, including the following:
- Describe the importance of leadership for compliance with the initiative.
- Discuss how the initiative will improve the healthcare environment.
- Describe strategies leadership can use to maintain compliance with the initiative.
- Describe one current or emerging technology that will support safe and effective implementation of the changes. Examples may include electronic alerts within the EMR, electronic bladder stimulation devices, biofilms, etc.
Part III: Course Reflection
Submit a 2-3 page course reflection paper. Reflect on the six course competencies:
